The Department of Justice has refused to detail disciplinary action taken after youth custodial and riot squad officers swamped, pinned down with shields and shackled a 16-year-old detainee after he threatened self-harm.

Explosive footage of the incidents at Banksia Hill and Unit 18 last year, exclusively obtained by The West Australian, also showed the troubled teen cry out while being restrained in the now-banned “figure four” hold.
